Welcome to Yael Center

The Yael Center is a professional therapeutic organization which focuses on speech therapy and multi-dimensional therapy via two unique methods, the VML and MDT methods accordingly. It is called Yael in honor of one of the first children Dr Vashdi treated at the beginning of his professional way. The idea to choose this type of name to the clinic came from the deep understanding that the knowledge and service are being developed for the children and not the profession. The center has been in existence since 1996.

When observing the VML treatment from the outside, the most visible element is the use of manual techniques.

300 manual techniques help the child to find the motor patterns of speech quickly, serve as a trigger for sound production and sometimes are crucial in the learning process.

All the techniques were developed through practice and are based on motor control parameters of each phoneme. Each technique has physiological explanation and should be used according to the source of speech mistake.

This is the easiest element to control with the VML method training hence the first element taught.

Dear colleagues and families,

It is highly important to acquire all the theoretical knowledge regarding the speech treatment – phonetic knowledge, anatomy and physiology or sensory systems, it is very important to control all the manual techniques and to be able to analyze correctly the speech condition in order to set the most accurate speech treatment.

However, applying the program is a different story and skills set. For that purpose, we have developed a set of principles which guide us through the challenges of speech treatment with the non-verbal child.

One of the most important principles is the platform principle which sets the frame for practice. The platform is a preferred activity by the child through which the therapist can apply the treatment goals in the most effective way

The online version training of the VML method evolved during covid time and proved to be efficient. The student presented new significant achievement in treatment in a few months, extremely impressive 😊

The tables of sounds started to be filled quickly due using the correct programs and techniques. Non verbal children started talking and advanced to the language phase.

The VML methods was starting to develop 28 years ago for the treatment of verbal Apraxia. Over the year the system grew and evolved through practicing with hundreds and thousands of children annually.

The system was developed by Dr Elad Vashdi , manager of yaelcenter institute, therapist and researcher in the area of pediatric rehabilitation.

The system includes 4 major areas – algorithmically evaluation and analysis, 300 manual techniques, 20 motor learning principles and 20 unique treatment principles. The integration of these areas in every point of the treatment is the VML treatment at its best. The more variables integrated in every point the more accurate the treatment will be.

The training course is designed to deliver the basic knowledge of the VML method and train the practitioner to use the knowledge in the field.

This week we had two major VML training events:

End of the Taiwan June 2023 practical training
Second section of the Romanian VML training - Dec 2023.

The Taiwan group of speech therapist show great skills and results already. They will soon finish their assignments towards the VML therapist certificate.

The Romanian group went through the 2nd section so quickly that it seems like we didn't have enough time. It is a big talented group with international participants from Holland, Spain, Portugal and Israel.

I want to thank all the participants for their passion to help the children. Big thank you for my colleagues in Taiwan and Romania - Elaina, Panny, Raluca and Paula for being such an amazing partners 🙂

Last week I had an extraordinary two intensives for a whole week. one of them was with a young brilliant guy, not walking yet due to severe ataxia and non verbal. It was our third meeting - first time intensive. The meeting exceeds all expectations......

Here is a summary of the meetings - a little bit long but very interesting ...

The boy was re-evaluated for VML treatment 18 months ago and a program was set. This time we met for an intensive therapy of 8 days, 2 meetings a day of 2 hours each.
The goals of the intensive treatment were:
1. Re-evaluating the speech skill and setting a new program
2. Gaining new sounds and better control over speech sounds
3. Support any other skill required.

The boy presented extraordinary control over the computer, designing 3d models, engaging in many online activities, solving tech problems on the phone and computer, advanced quickness in operating software despite the hands and whole-body spasticity. Wow. He advanced dramatically with his motor control and function, although still not walking nor standing.

The meetings were very effective, and we were able to gain many achievement in 4 main areas:
1. Breathing control and meditation
2. Speech
3. Typing and expressive language
4. Lower extremity motor control

Breathing control and meditation
Breathing volume was found low as well as shallow and sharp. The breathing pattern affected the speech, motor behavior and emotional status. We practiced adjusted Jacobson meditation, diaphragmatic breathing and guided imagination in several ways.

The practice was very successful with an immediate effect. Motor control improved in a few tasks, speech was more controlled, and we had a mental tool to use when control is low.
We practiced breathing exercises using breathing training tool with 3 balls for inhalation and exhalation, alongside other exercises.

Speech
We practiced single syllables consonant group based and vowels group based, increased number of consonants and syllables, using unique strategies to control the speech patterns overriding the spasticity. Towards the end we were able to practice words at CVC and CVCV structure! We used reaction lights to measure response time for syllables and words production. Extraordinary, truly. Typing and expressive language
He is not using typing as a communication tool but rather voicing and gestures. Quick examination presented a very surprising result – copying words is very good and quick while the ability to retrieve words in typing is compromised to the level of short and known words. The inability to retrieve words in typing blocks completely the ability to communicate in this way. This skill wasn’t practiced specifically or identified as a deficit until now.

A designated practice during the intensive program brought an immediate improvement and demonstrated the problem more thoroughly.
Being able to retrieve nouns and verbs in typing will open a very significant channel for communication, learning and engaging socially. It is highly important to be accurate and focused on the threshold of this skill to enable the quickest results.

Lower extremity motor control
The boy was practicing motor control most of the time in therapy through out the years. The main goal was to achieve standing and walking. After examination and observation, it seems like in order to move forward at the moment we need to isolate the control over each main joint – mainly knee and hip in various postures.

There are movement strategies already acquired and emotional stress accompanied. We focused on knee standing exercises – strength, balance, movement control using breathing strategies. We used interesting tools such sally up and down song at 0.75X speed, metronome pacing and timer.


Designing
Not part of the practice but a very important section of the global picture. The boy has a unique skill and interest in 3D designing which can potentially compete at a typical professional level. He is spending most of his time learning this field, participating, and improving his skills. I would recommend investing in the area as a future profession, setting a learning environment for him to grow professionally under professional guidance.

2. Gaining new sounds and better control over speech sounds
3. Support any other skill required. 

The boy presented extraordinary control over the computer, designing 3d models, engaging in many online activities, solving tech problems on the phone and computer, advanced quickness in operating software despite the hands and whole-body spasticity. Wow. He advanced dramatically with his motor control and function, although still not walking nor standing.
 
The meetings were very effective, and we were able to gain many achievement in 4 main areas:
1. Breathing control and meditation 
2. Speech 
3. Typing and expressive language 
4. Lower extremity motor control

Breathing control and meditation
Breathing volume was found low as well as shallow and sharp. The breathing pattern affected the speech, motor behavior and emotional status. We practiced adjusted Jacobson meditation, diaphragmatic breathing and guided imagination in several ways. 

The practice was very successful with an immediate effect. Motor control improved in a few tasks, speech was more controlled, and we had a mental tool to use when control is low. 
We practiced breathing exercises using breathing training tool with 3 balls for inhalation and exhalation, alongside other exercises. 

Speech 
We practiced single syllables consonant group based and vowels group based, increased number of consonants and syllables, using unique strategies to control the speech patterns overriding the spasticity. Towards the end we were able to practice words at CVC and CVCV structure!   We used reaction lights to measure response time for syllables and words production. Extraordinary, truly. See comparison between tables. 

Typing and expressive language 
He is not using typing as a communication tool but rather voicing and gestures. Quick examination presented a very surprising result – copying words is very good and quick while the ability to retrieve words in typing is compromised to the level of short and known words. The inability to retrieve words in typing blocks completely the ability to communicate in this way. This skill wasn’t practiced specifically or identified as a deficit until now. 

A designated practice during the intensive program brought an immediate improvement and demonstrated the problem more thoroughly. 
 Being able to retrieve nouns and verbs in typing will open a very significant channel for communication, learning and engaging socially. It is highly important to be accurate and focused on the threshold of this skill to enable the quickest results.  

Lower extremity motor control
The boy was practicing motor control most of the time in therapy through out the  years. The main goal was to achieve standing and walking. After examination and observation, it seems like in order to move forward at the moment we need to isolate the control over each main joint – mainly knee and hip in various postures. 

There are movement strategies already acquired and emotional stress accompanied. We focused on knee standing exercises – strength, balance, movement control using breathing strategies. We used interesting tools such sally up and down song at 0.75X speed, metronome pacing and timer. 
 

Designing 
Not part of the practice but a very important section of the global picture. The boy has a unique skill and interest in 3D designing which can potentially compete at a typical professional level.  He is spending most of his time learning this field, participating, and improving his skills. I would recommend investing in the area as a future profession, setting a learning environment for him to grow professionally under professional guidance.

From one of the last evaluations in Romania. Again a case of severe motor condition to which we were looking a solution.

Tetra plegia and blindness, no speech. So no option to communicate verbally or through using the eyes in a yes/no system.

However, there is understanding of language and will to communicate.

One of the conclusions from the meetings with this type of population is that there is a huge gap between the motor condition and the cognitive potential and understanding. However , most if not all therapy time us invested in the motor treatment and almost nothing on the cognitive/language/communication side, even though there is more potential there and it is as important. The cognitive skill is locked in a safe... We need to open the safe.

In this case we will build a yes/no communication system based on mouth movement. Open mouth for yes, closed mouth for no.

From one of the last intensives - practicing the flexibility of language, creativity, coherence, sequencing, syntax and vocab through writing a story together. Each contributed a sentence or an idea in his/her turn.

From one of the latest evaluations of children without mobility or speech - a yes/no system based on eyes movement.

Goal – build a yes/no system using the eyes.
The child would be able to use the eyes to say reliable yes/no answers for questions related to his will and simple informative questions.

Pre-requisite skill – eyes tracking

Practice eyes tracking in three directions:
1. Side to side
2. Up and down
3. Back and forth

Hold something the child likes and move it in three planes. Make sure tracking is fast enough, stable, consistent and fluent.

Stages of learning

1. Id one card with the eyes – show one card and ask Darius to look at it. Put it in different locations
Practice 10 different objects.

2. Choose one card – present two cards and ask for one. Example – ball and hat. Put them apart 50 cm, ask only for the ball 3-5 times and change locations. Do the same for the hat.
Work on 10 words.

3. Choose one card by will – what do you want to eat? Give two options with cards.

4. Teaching yes/no concept – present to cards that represent yes and no by colors. Red for no , green for yes. Ask 5 yes Q on objects and direct him to look at the green card. Show an apple – is that an apple? Ask 5 no Q on objects, direct him to look at the red card. Show an apple – is that a pen?

5. Ask yes/no questions regarding his will. Find 5 things he really wants and 5 he really doesn’t want. Present first the things he wants through a yes/no question and direct him to look at the green card. Do the same for the things he doesn’t like and direct him to look at the red card.

6. Ask a yes/no informative Q regarding an object. Ask 2-4 times a ‘no’ Q and then a ‘yes’ one. Example – hold a car – is it a flower? Is it s balloon? Is it a car? Make him look at the right card as an answer.

7. Ask a yes/no informative Q regarding a simple 1-2 sentences declaration. Declare 1-2 sentences and ask regarding them. Example – the brown horse ran in the park? Did the horse ran on the road? Was it a horse who ran in the park? Was the horse brown? Etc.

8. Use the yes/no eyes based system to teach new materials. Introduce knowledge in various areas to your choice and test the knowledge using yes/no questions.

Even in such a difficult condition of only the eyes muscles are
working we can find a way for significant communication and learning.
